The Future Role of Patient Data

The Role of Patient Data for Patient-Centred and Integrated Care

Patient data come from different sources and are exchanged and used for different purposes. The amount of data and the parties involved in the exchange will increase dramatically. The presentation gives a brief overview of the use of patient data from providers’ perspective as well as from the angle of the HIMSS Analytics Continuity of Care Maturity Model. Following this, a concrete example of a Dutch hospital and its regional partners will illustrate how patients can be actively involved in the care process today and which concrete plans and strategies are being pursued for the future.
